insurance-page-photo.pngIf we have a contract with your insurance company, your co-payment, if applicable, is due at the time of service, prior to being seen, and cannot be waived. We will not bill for co-payments. Failure to pay your co-payment at the time of visit will result in a $25 surcharge.

If your insurance plan requires a referral, you will need to contact your primary care physician and request a referral, prior to scheduling your appointment with our office.

Our office participates in most major insurance plans, and as a courtesy to our patients we will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account and for knowing the requirements as well as the limitations of your insurance plan. We encourage you to contact us if your policy has not paid within 30 days. Please refer to our Financial Policy for a list of payers we currently do not accept.
