About Your Visit


Before you arrive, you’ll be emailed an invitation to join our online patient portal (powered by healow™) and complete a health questionnaire. This allows our providers to review your personal medical history, family medical history, and any current health concerns you may be experiencing. Plus, by completing this information on the portal from home, it saves you time on the day of your appointment.

If you aren’t able to provide this information via the portal prior to your appointment, you’ll be asked to complete this paperwork digital using our touchscreen kiosk tablet. Completion of this information at our office can take some additional time, so we ask all new patients to arrive 30 minutes prior to their scheduled appointment time if this information wasn’t completed on the portal.

On the day of your appointment:

  • Remember to bring a photo I.D. and your most current insurance card(s).
  • Please arrive early, allowing ample time for traffic, parking and completing any necessary paperwork that wasn’t completed on the portal.
    • New patients – 30 minutes
    • Established patients – 15 minutes
    • Procedures – 45 minutes
  • Please bring all test results, lab work, X-rays, etc., that pertain to the reason for your visit. If you do not have these documents, please make sure they are mailed or faxed prior to your appointment.

Patient Forms & Policies

To assist us with your care, please bring a list of all current medications you are taking with you to your appointment. For you convenience you can download and print our Medications Form and fax to (912) 790-2701 prior to your appointment.

Medical Records Release

If you need to request or receive your medical records from your physician, please print and fax the completed Authorization to Release/Receive Medical Records to (912) 790 -2701.
